Financial Aid Printable Forms

Once we have completed your award package you will receive an e-mail notification letting you know that it is ready for viewing in the "My Financial Aid" link of the SMC Student Portal. There is also a link for "Document Tracking", where you should find which of the documents below you need to complete and any other documentation we may need from you to complete the award process. All documents will be listed by the department in which they are needed. If you have questions concerning your Award Letter or Document Tracking you may contact the Financial Aid Office. State Aid Certification:
State Aid Affidavit - (To be printed and completed by all recipients of SC LIFE Scholarship and SC Tuition Grant)

United Methodist Recommendation Letter/Form:
United Methodist Recommendation - (To be printed by United Methodist church members and completed by pastors as recommendation for the SMC United Methodist Scholarship of $1000)

Verification Worksheets:
Dependent Student Verification Worksheet - (To be printed and completed by all FAFSA filers whose FAFSA is selected for verification; dependent students are those who report parental information on the FAFSA)

Independent Student Verification Worksheet - (To be printed and completed by all FAFSA filers whose FAFSA is selected for verification; independent students are those who are over 24 years of age and/or do not report parental information on the FAFSA)

 

Loan Forms:
SMC Parent Loan Request Form   - (To be printed and completed in full by a parent wishing to borrow PLUS funds during the 2011-2012 academic year)

Self-Certification Form  - (To be printed and completed in full by a student or parent wishing to borrow alternative/private loan funds during the 2010-2011 academic year.  This form must be returned to the lender, not the SMC Office of Financial Aid)

Special Conditions and Unusual Circumstances:
Dependent Student Special Circumstances - (To be printed and completed by students and parents who may need to request further financial review because of special or unusual circumstances that have occurred in the family that the FAFSA does not accurately reflect)

Independant Student Special Circumstances - (To be printed and completed by students [without parents] who may need to request further financial review because of special or unusual circumstances that have occurred in the family that the FAFSA does not accurately reflect)

Request for Dependency Evaluation - (To be printed and completed by students who would ordinarily need to provide parental information on the FAFSA, but due to unusual circumstances are appealing to report only student information on the FAFSA)

Clarification of Assets - (To be printed and completed by families, at the request of the Office of Financial Aid, who may have omitted certain questions on the FAFSA or may be undergoing a special circumstances review.)

Low Income Statement   - (To be printed and completed by families, at the request of the Office of Financial Aid.)
